Liability Release Form

This must be filled out prior to your in-person course.

It is now required by the American Association of Veterinary State Boards (AAVSB) that CE providers (that’s us!) report the licensing information of our participants.

Never lose track of what you need.

If you made the purchase under your own account, you will be auto-enrolled in the course.

If your office manager or other administrative staff member at your practice purchased the course for you, make sure to create an account for yourself, if you haven’t already. Then email [email protected] with your name and the course you should be enrolled in. Our staff will add you to your course.

Please follow these steps on how to start the course and, following its completion, where to access your certificate.

  1. Begin by logging into your account (under your name) at https://csuvetce.com
    • Important information about your enrolled courses is accessible from here.
  2. To receive certification and RACE credits, you must complete all portions of your online and in-person components.
    • Please note that all in-person courses have online components.
    • Each course has a completion task. Once you update your course as completed, your course completion certificate will be available to download.
  3. Please submit the post-course survey. Your feedback is valuable to the continual success of our program.

Course Access Policy: 

Access to this course will expire one year (365 days) from the course’s end date. After this period, users will no longer have access to course materials, resources, and any associated content. It is the responsibility of the users to complete the course and download any materials including their course completion certificate.

Attire

Casual–learning is done best when you’re comfortable. We recommend bringing a sweater or light jacket, as the TMI runs a little cold.

Scrubs will be provided each day of your course. These do need returned before you leave. The TMI also provides shoes to borrow (Crocs).

Meals

Meals are provided for all registered participants. Depending on your scheduled course time, this could include breakfast and lunch.

Luggage

The TMI does have luggage check on site for all participants (at no charge).

Badge Access

All participants will receive visitor badges for access to locked CE areas. Please keep your badge on at all times and return to staff before you leave the facility on the last day of the course.

Cancellation and Refund Policy

We offer 100% refunds for CSUVetCE courses until 60 days prior to the course start date.

50% refunds or credit until 30 days prior. Cancellations due to illness or unforeseen circumstances will be considered on an individual basis.

0% refund within 30 days of a course start date. However, depending on the circumstance, you may qualify for credit that can be applied towards another CSUVetCE course offered within the same calendar year.

Course Waitlist

Never miss a course!

We release courses on a quarterly basis (check our course catalog to review this year’s release dates). If you see a course you want to attend but registration hasn’t opened, get on the course waitlist and we’ll notify you when you can register.

All CSUVetCE participants receive a complimentary parking pass. Instructions and a coupon code for your parking pass will be located in your account’s course materials, and is typically released one month prior to the course start date.

If you’ll be a part of a group that is using a bus service, no parking information will be needed.

The TMI is open from 7 a.m – 4 p.m. Hours outside of these require a badge to enter. You can only enter the building on weekends using a badge.

Paid parking is enforced Monday – Friday (8 a.m. – 4 p.m.) with free parking on the weekends.
